How To Return Equipment for Repair
  1. Please contact our Customer Service Dept. at 408.748.7800 ext.159 or technical support, , to request an RMA form. The RMA form can also be filled out and submitted online, (see below.)
  2. Fill it out, then fax it to Customer Service prior to shipment to 408.748.7801.
  3. Once the request is received, a Customer Service Representative will contact you with an RMA number.
  4. Once you have received your RMA number please ship the equipment prepaid to the following address:

    Attention: Repair Department
    Please ensure that a copy of the RMA Form or RMA information is included in the box(es) with the returned equipment. Please do not send units without an RMA number.

If the product(s) is found to be out of warranty, there is an initial cost of $250 for evaluation and testing. A cost estimate will be sent to the customer for the repair, at which time, we will require a Purchase Order to proceed.
